Nature of telehealth
Care is provided remotely using written intake information, uploaded photos and documents, and laboratory results, rather than an in-person physical examination.
Limitations
Remote evaluation cannot detect everything an in-person exam would. Some conditions cannot be treated safely this way, and you may be referred for in-person care.
Risks and benefits
Benefits include convenience and access. Risks include delays in evaluation, and clinical decisions made on incomplete information if what you report is inaccurate.
Your responsibilities
Answer honestly and completely, report new symptoms and side effects promptly, and complete recommended labs and follow-up.
Right to withdraw
You may withdraw consent and stop treatment at any time, including by requesting cancellation from your dashboard.
