This describes how your health information is handled. It is a draft and must be reviewed by counsel before launch.
What we collect
Account details, the answers you give in an intake, documents and photos you upload, and the record of your treatment requests and their status.
How it is stored
Records are held with per-patient access controls. Your intake, requests, and documents are visible to you and to the clinical team only. Other patients cannot access them.
How it is used
To evaluate whether treatment is appropriate, to prescribe and route prescriptions to a pharmacy, and to follow up on your care.
Sharing
Information is shared with the dispensing pharmacy and with laboratories where testing is ordered. It is not sold or used for advertising.
Your choices
You may request a copy of your records, ask for corrections, or request deletion of your account. Some records must be retained for the period required by medical record law.
